At this time, the New Patient Forms must be downloaded and filled out manually. However, if you wish to scan and send them via email before your first appointment, you may do this at your discretion. The same is true for any future documents you wish to send me. Please use this email address for attachments: email@example.com.
The e-mail address firstname.lastname@example.org, is capable of sending and receiving encrypted email as well as encrypted attachments, but that process has to be initiated by me. If you send me an email to this address, it will NOT be encrypted. (If I first send you an encrypted email, and you reply from within it, it will be.) Even then, no email program is guaranteed to completely guard your privacy. If you become a client, you will also be able to send and receive e-mail via TherapyAppointment.com, which uses encryption and is password-protected. For security reasons, however, that address does not accept attachments. Also, because texting is not secure, I do not text with clients. Please do not use Facebook for communication of personal information, in order to guard your privacy.

If you would like to schedule your first appointment, or talk to me to learn more about my practice or style of work, please call the office at (469) 219-3311 and leave a voice mail message. I will return your call during business hours within the next 24-36 hours, or on Monday if you call on Friday afternoon or during the weekend. You may also send an email to: email@example.com. Please be cautious with email and do not send detailed personal information in this manner. I also have an assistant, Judy, who can return calls regarding schedule changes and billing or insurance questions, but not schedule new clients.
After the first appointment, you will be entered into the TherapyAppointment.com database with a unique log in and password. This will enable you to go online 24 hours/day to set future visits, cancel an appointment (with more than 24 hours notice), see available appointment times, edit your personal information, or make a payment. I do not currently see clients on the weekend.
PLEASE NOTE: IF YOU ARE 20 MINUTES OR MORE LATE FOR AN APPOINTMENT AND HAVE NOT CALLED TO NOTIFY ME YOU ARE RUNNING LATE, THIS WILL COUNT AS A NO-SHOW AND YOU WILL BE CHARGED $100.00.
SCHEDULING AN APPOINTMENT

I am an in-network provider in McKinney for Blue Cross and Blue Shield PPO plans, Cigna, Tricare, Medicare, Aetna and some small providers. Most other insurance companies will consider me to be 'out of network.' This means they are likely to pay between 40 and 60% of the allowable cost of your visit. I am happy to provide you with the necessary paperwork if you wish to file for reimbursement from your insurer. I am not an EAP provider, even if your insurance company tells you that I am!
All services are payable at the time of visit. I accept credit cards, cash (exact change please) or personal checks. VISA, Mastercard, Discover, health savings accounts and American Express cards are all accepted. I also ask that you keep a credit (or debit) card on file with me for payment of any missed appointments or miscellaneous charges.
If you use insurance you will likely have a deductible. These start over at the beginning of the "plan year" (which may or may not be January 1). Please be aware that you will be responsible for paying the deductible. In addition, insurers may consider psychologists to be specialists, so the copay amount could be the specialist charge. If you have questions about your coverage, please call the number on the back side of your insurance card and ask the representative.
Also, please remember to bring a photo ID and your insurance card to the first appointment! All minors must be accompanied by at least one legal parent or guardian at the first visit.
Because therapy reserves a significant block of professional time just for you, I ask that you provide a 24 advance notice for all cancellations. You may call (469) 219.3311 if it is less than 24 hours before your visit and leave a message. This is considered a 'late cancellation.' If you do not call and do not keep your scheduled appointment, that is considered a 'no-show.' As described in the documents which may be accessed elsewhere on this site, there will be a charge for either a late cancellation or a no-show.
